The Form and the Fee

Utah's change filing for LLCs is the Limited Liability Company Registration Information Change Form. It has no form number; the state does not assign one. Find it on the Division's registration information change forms page. Processing fee: $17, non-refundable, same amount online or on paper.

Four Steps

1. Pick a qualifying agent: over 18 with a Utah street address or rural route box if an individual, authorized in Utah if a company.
2. Fill out the Limited Liability Company Registration Information Change Form with your entity name, entity number, and the new agent's details.
3. Send it to the Division of Corporations and Commercial Code with the $17, or bundle it into your renewal and skip the fee.
4. Pull up your entity in the state's business search and confirm the new agent shows.

Transition Notes

Documents can keep arriving at the old agent address until the Division records the update. If the outgoing agent was an officer using a home address, have them watch the mail through the switchover. After the record updates, everything routes to us.
